MARATHON HEALTH - CENTER FOR HEALTH AND WELLNESS
STPH has partnered with Marathon Health to run the Center for Health and Wellness, a free benefit offered to employees and spouses participating in the hospital’s medical insurance plan. The Center, led by licensed Nurse Practitioner Tiffany Houser ANP-C, works closely with each Primary Care Provider to ultimately improve the health of the workforce and contain health care costs for the organization. Services offered focus on prevention and disease management, and can cover a large variety of topics.
In addition, we are pleased to announce that beginning Sept. 2, 2014, the Center for Health and Wellness will also become a resource to STQN members to help achieve population health management goals for the STPH population. Using HEDIS quality measures, the Center will offer patient-specific interventions geared toward accomplishing optimal outcomes between physician visits. These interventions ensure all required screenings are performed, regularly monitor and document required HEDIS metrics, provide on-going disease state education and ensure compliance with prescribed medications.
Diabetes is the first chronic condition the Center will manage with STQN. HEDIS Care Measures monitored by the Center include Hgb A1c, eye exams, LDL, blood pressure and nephropathy. The Nurse Practitioner regularly interacts with the patient to maintain the required metrics and reports all interactions to the physician for documentation in the medical record.

To refer a Gilsbar 360 (STPH employee or their spouse) for the Diabetes Management program, please fax patient name, DOB and contact number to (877) 769-0320. Once received, a service coordinator will contact the patient and facilitate scheduling the appointment. If desired, they can send a note back to the physician’s office informing them of the appointment day and time.
